Restasis (cyclosporine), a brand-name eye drop for chronic dry eyes, can be expensive. According to Optum Perks, one bottle of multidose Restasis can cost as much as $700.
Medicare Part D plans may cover Restasis. But whether or not your plan covers this drug depends on your specific plan. How much you’ll pay for it out of pocket depends on the drug’s tier in your plan’s formulary.
That said, there’s a way you may get Restasis entirely for free. In this article, we’ll explore how the Restasis patient assistance program works and other ways to help you lower drug costs if you’re enrolled in Medicare.

AbbVie manufactures Restasis and offers a patient assistance program called myAbbVie Assist. Under the program, if you fulfill the eligibility criteria, you can get Restasis for free for an entire year.
You can qualify for the program if you fulfill any of the following criteria:
- You are under the care of a licensed U.S. healthcare professional and have been prescribed an Abbvie drug that’s part of the program, such as Restasis.
- You have little or no health insurance coverage.
- You have limited economic means.
- You reside in the United States.
Note, however, that if you qualify under any of these criteria while insured by Medicare Part D, you won’t be using your insurance to purchase Restasis.
To apply for Medicare’s Restasis patient assistance program, you’ll need to fill out an application. Review the participation terms, privacy notice, financial details, and Health Insurance Portability and Accountability Act (HIPAA) permissions on pages 1 to 3, then print and fill out the enrollment form on page 4.
Remember to consent for eligibility verification by ticking the boxes in Section 5 and signing along with the date to show you understand the Terms of Participation.
You’ll also need to add a separate signature and date for HIPAA authorization. If you’re covered by Medicare Part D, you must include front and back copies of your insurance card. You can then submit the application online, by fax, or by mail.
myAbbVie Assist is a manufacturer assistance program. You also have the option to apply for the Extra Help program, also known as the Part D low-income subsidy.
In 2025, Extra Help covers your Part D premium and deductibles fully, and you’ll only pay up to $4.90 for generic drugs. You won’t pay more than $12.15 for brand-name drugs.
If you’re considering joining myAbbVie Assist, there’s an important point to keep in mind. That is, if you’re eligible for Medicare’s Extra Help, you won’t be eligible for the Restasis patient assistance program unless Extra Help denies you first. You’ll have to provide documentation of this denial as part of your application.
